Our Hospice at Home service provides compassionate nursing care for people in the last 4 weeks of life, enabling them to remain in the comfort and familiarity of their own home wherever possible.
Care is delivered by a team of experienced Registered Nurses and Senior Healthcare Assistants who are specially trained in end-of-life care. We work closely with patients, families, and other healthcare professionals to provide personalised support that meets each individual's needs.
We offer hands-on nursing care, emotional support and practical guidance for patients and those important to them. Together with your GP, district nurses and other health and social care professionals, we help manage symptoms and ensure care is coordinated and responsive.
Our service includes:
Personalised end-of-life care in the patient's home.
Emotional support for patients, families and carers.
Support with symptom management, including pain relief and comfort measures.
Advice, reassurance and guidance when concerns arise.
Close working with community palliative care teams, district nurses and GPs to ensure seamless care.
Routine care visits occur between 8.00am and 8.00pm, with limited overnight support available. We offer once or twice a day visits – unlike a care agency these are not time restricted so we can adapt to changing needs.
As a person's condition changes, we work with patients and their family to agree the most appropriate level of support, focusing on both physical comfort and emotional wellbeing. The Hospice at Home team works with other healthcare professionals to ensure the right support is in place.
Where available, we may be able to provide overnight care, allowing family members and carers time to rest while knowing their loved one is being supported by a member of our team.
This is decided daily according to the needs of all the patients we care for. We work with Marie Curie to provide care at night.
24-hour telephone support is available via the advice line on 0117 915 9430, ensuring that patients, families and carers can speak with someone at any time. Whether you have questions about symptoms, need practical advice or simply want reassurance, we are here to help.
Patients may be discharged from Hospice at Home if care needs become longer term and care may be transferred to a Continuing Healthcare Fast Track (CHC FT) package if appropriate to do so. This will be discussed prior to any changes coming into effect.
Our aim is to provide high-quality, compassionate care that helps patients spend the last short weeks of their life as comfortably and as well as possible at home, while supporting those closest to them throughout their journey.
