How to make a referral

A request for assessment can be made by any professional, but must be with the GP's consent. The referral should be directed initially to the Medical Secretary, St Peter's Hospice at the address shown below. 

There is a flexible referral package available so that an appropriate assessment can be made of individual patient needs. The initial assessment visit may include any one or a combination of:

  • A Hospice Community Nurse Specialist
  • A Hospice Doctor
  • The patient's GP
  • The District Nurse
  • The Lymphoedema Sister

To make a referral:

  • Contact the Medical Secretary at:

  • Telephone 0117 915 9225 and ask for the Medical Secretary, who will arrange for a referral form to be posted to you for completion

If the patient is not already known to the hospice an assessment visit may be necessary by a member of the hospice team before the service is introduced.
