What can we offer you ?

The Social Work team provide high quality social work services to patients staying at the in-patient units or visiting the Day Hospice at Knowle or Brentry. A large part of the work is supporting patients and their families in the community. We work closely with the Community Hospice Team and other professionals both at the Hospice and within other agencies

We can help with :

  • Emotional and psychological support
  • Helping patients and families plan for discharge from the In-Patient Unit
  • Advice and support for families needing to find appropriate nursing homes to care for their relative
  • Organising practical support and Home Care for patients wishing to leave the Hospice and continue to live independently in their own homes
  • Offer advice or information about DSS benefits, making wills and financial problems
  • Act as representatives for patients, carers and their families
  • Liaise with Social Workers and other professionals within community Social Services and Health Service Teams
  • Reminiscence work
  • Arrange visits by our volunteer Family Support Service

F.L.A.G.S. (Family Liaison and Grief Support)

Provides support for children and families
Ways in which we can help:

  • Preparation for the death of a loved one
  • Support for the family through the process of grieving
  • Work independently with children and young people, experiencing illness, loss and bereavement of a significant adult, on a one to one basis or in small groups
