At Home


We have four teams of Community Nurse Specialists who visit patients in their own homes. We provide expert advice and support to the patients with conditions, usually cancer, where curative treatment is no longer possible. Support is provided to the patient, families, carers and primary health care teams by working in partnership with them and other agencies involved.

What can you expect from us ?

  1. Support for you, your partner, family and friends by phone or visits to your home
  2. Specialist advice about any symptoms you may experience
  3. Access to specialist medical support and all hospice services
    e.g. Day Hospice, In patient care, support for children
  4. Specialist advice for anyone involved in your care
  5. Help and advice with financial or social problems
  6. Liaison with other agencies involved in your care and other services that may be able to help

We work closely with your GP and District Nurse to help provide the best care possible.

If you do not wish us to discuss your care with your partner, friends or family without you being present, please let us know.

We aim to help you live as full a life as possible.
